Procedures
In order to obtain University departmental flat sheets, complete a departmental flat sheet order form (ECom login required). When completing this form, please indicate whether the order is new, a reorder without changes, or a change order.
Proofs are provided in Adobe Acrobat (PDF) format via e-mail. (Hardcopy proofs and proof cards are available upon request for those who do not have e-mail access.) It is the client's responsibility to carefully review the proof and mark any necessary changes. Clients are provided with the option to sign off on a project as-is or with changes as indicated. A client's explicit approval (via e-mail or signature) is required before a project is printed; a client is charged for all reprinting that results from errors that were not discovered on an approved proof.

Turn-around time is approximately 2 weeks for new flat sheets and 2 to 3 days for reorders without changes.
Prices
Admissions mandates that departments update all field of study undergraduate flat sheets each school year. Admissions will pay for the first 150 copies ordered by a department that updates its flat sheet and qualifies. DPS charges a setup fee (also covered by Admissions for departments that qualify) in addition to production costs for the first 150 copies. After the first 150 copies, an additional setup fee will apply to any order requiring changes to the content of a flat sheet.
