Speech Language Pathology Assistant
Individuals with a masters in speech-language pathology who are participating in a supervised professional experience and hold a temporary license in the state where they provide services to Illinois children may apply for an EI Associate: Speech/Language Pathologist in supervised professional experience credential. Associate services are billed under the enrolled supervisor's name. Credential required, CBO enrollment not required.
If you have completed your Clinical Fellowship Year (CFY) and have applied for and been issued a Speech Language Pathologist license, you need to apply for the Early Intervention--Speech Language Pathology credential. Please submit the following:
- The first page of the EI Credential Application
- A copy of your current license
- A completed EI Central Billing Office Enrollment Application.
Speech/Language Pathology Assistants with a current license in the state where they provide services to Illinois children may apply for an EI Associate: Speech/Language Therapy Assistant credential. Associate services are billed under the enrolled supervisor's name.
