Provider Connections

All transportation providers must enroll in the Illinois Medical Assistance Program Inninois Medicaid Program Advanced Cloud Technology (IMPACT) System.

Individuals and/or Agencies with an appropriate vehicle registration number, insurance, and current drivers license may enroll to provide to bill for transportation services provided to EI children and families.

Individuals may enroll to be reimbursed for costs incurred as a result of transporting their child to/from EI services identified on an IFSP. Individuals wanting to enroll this way should indicate on the EI Service Matrix that they are parent transportation providers so they will be identified in the system as such and not contacted to provide transportation services for other children.

Provider Agency may enroll the agency as a whole to provide transportation services.

Provider Connections processes the initial enrollment documentation for transportation provider enrollees and forwards the application to the EI Central Billing Office (CBO).
