Join students from many countries for the 6th annual Western Illinois University
Summer Program 2009 - Sustainable Entrepreneurship and Community Commitment, which will provide (1) the opportunity to expand your knowledge to develop new business models in the ever-changing marketplace today; (2) a
seminar taught in English and in Spanish ; (3) participation in an intensive
English Language Course at the level of proficiency (beginner, intermediate or advanced after assesment);
(4) knowledge of local customs and history; and (5) many cultural activities,
including several excursions.

The Summer Program 2009 is open to any university student over the age of 18 who is interested in gaining current knowledge and experience of the American culture. All participants of the Summer Program 2009 must have a J1-Visa or F1-Visa. All applicants must be university students, either undergraduate or graduate, from any academic discipline. No applicants under the age of 18 will be accepted during any part of the Summer Program 2009. Proof of university enrollment and age will be required upon registration. We suggest you apply as early as possible in order to secure your place in the program.

To apply to the Summer Program 2009, you must complete all the application
forms.
All completed application forms must be accompanied by a $25.00 nonrefundable
registration fee. The full program fee of $2,000 is due immediately upon the
receipt of your appropiate Visa and payment in full must be received prior to arrival.
We will accept applications up to April 15th, 2009. Applications received after
this date will be reviewed on a case by case basis and evaluated based upon availability
and the possibility of obtaining your visa in a timely manner. You should not
assume that you have been accepted. Wait for confirmation from us before making
further arrangements for travel, etc.
Once you are accepted for the Summer Program 2009, you will receive confirmation via
email and will be provided with detailed information about the program. The information
will include arrival instructions, details about the Entrepreneurship
and English courses, tentative itinerary of activities, suggested reading lists,
and a list suggesting what you need to bring.

You are required to complete the following application forms:
IMMUNIZATION
Western Illinois University’s Immunization policy reflects the Illinois College Student Immunization Act (Copy Enclosed) which requires International Students to document the following:
1) Dates of any combination of three or more doses of Diphtheria, Tetanus, and
Pertussis (DTP or DTaP), pediatric Diphtheria and Tetanus (DT), or adult Tetanus
and Diphtheria (Td) vaccine, with the most recent dose having been received
within 10 years prior to the term of enrollment.
2) Dates of 2 doses of live measles virus vaccine on or after the first birthday. The
minimum time interval between each dose must have been at least 4 weeks (28
days). If either dose was received prior to 1968, proof must be provided that a live
virus vaccine, without gamma globulin, was administered. Students who cannot
provide proof of immunization may provide laboratory (serologic) evidence of
measles immunity, or a physician's signed confirmation of disease history and
date of conclusive diagnosis.
3) Immunization with rubella vaccine on or after the first birthday, or laboratory
(serologic) evidence of rubella immunity. History of disease is not acceptable as
Proof of immunity for rubella.
4) Immunization with live mumps vaccine on or after the first birthday or a
physician's signed confirmation of disease history and date of conclusive
diagnosis.
5) International students are also required to have a tuberculosis skin test upon
enrollment or registration at WIU. In other words, the student must be tested
according to the Illinois Control of Tuberculosis Code at the Beu Health Center
upon arrival at WIU. Evidence of previous testing and/or vaccination with BCG
does not fulfill this requirement. Those students with a positive reaction to the
TB test will be required to complete a chest radiograph. A written report of a
chest radiograph will be accepted if completed within the three months prior to
the date of arrival in the United States.
Acceptable documentation means evidence of appropriate immunization, physician
diagnosed disease (if allowable), or laboratory evidence of immunity documented in
writing and signed by the physician or local health authority. To assist your students in
documenting their immunization status, a Beu Health Center Immunization Record is
included, with the required sections highlighted. A Health History form is also included
and should be completed by each student and returned with the immunization records.
Students without adequate proof of immunization will need to receive the required
immunizations, and may obtain them at the Beu Health Center by appointment. Estimated cost
for Td is $12.00, and the Measles, Mumps, and Rubella (MMR) combined vaccine is estimated at
$8.00; immunizations are not covered by student insurance. Currently, TB testing and
chest radiographs are free to the students.
Health Information and Documents to Download
The fee for the summer program 2009 is $2,000 and includes all academic instruction and materials for the English course and the daily semminars, transportation, housing, all breakfasts and lunches, cultural activities, and lodging while on excursions.
Please note that these costs do not include evening meals and miscellaneous/personal expenses. We suggest that each participant have a minimum of $500 for these additional expenses.
Students are responsible for the cost of their own airfare to and from Chicago, Illinois to their home country.

Payment of fees must be made by one of the following methods:
- Mail a cashier check's or money order in the amount of $25.00(US) for the registration fee (US). This payment must be sent along with your complete application package.
- Mail a cashier check's or money order in the amount of $2,500 (US). This payment must be sent immediately upon the receipt of your J1 or F1-Visa.
- Along with your check please include your name, address, phone number, and email address. You must provide a valid email address so we can notify you as soon as your payment is received.
Cashier's check or money order should be made payable to Western Illinois University. Do not send cash. Personal checks drawn on banks outside the United States cannot and will not be accepted. We also do not accept credit card payments of any kind.

If you cancel your registration prior to the beginning of the program, you will be charged the following:
A) Registration fee of $25.00 (nonrefundable)
B) Cancellation on or before June 1 will receive a full refund
C) Cancellation made after June 1 and prior to June 15 assessed a $250.00 cancellation fee
D) Cancellation made after June 15 receives no refund

We recommend that all participants purchase travel insurance before traveling to the U.S. This will cover any expenses incurred as a result of lost or stolen property, late arrival, early departure, or cancellation due to unforeseen circumstances. Please note that Western Illinois University is not responsible for the loss or damage of a participant’s personal property. This insurance should also cover medical expenses due to illness or unforeseen or unavoidable circumstances preventing or curtailing your stay.
All participants in the Summer Program 2009 are required to purchase Western Illinois University’s Health Insurance. This fee is included in your program fee.

WIU reserves the right to cancel the WIU Summer Program 2009 due to insufficient participants or circumstances beyond the University's control. Participants
enrolled by April 15, 2009, will be contacted by April 30th, 2009, and notified
of the cancellation.
