Q. Where can I download the CBO Enrollment Application?
A. The CBO Enrollment Application is located on the Provider Connections website at
www.wiu.edu/providerconnections. Click on "Enrollment" and then "Overview". There you will find the link for the entire CBO Enrollment Application

Q. Where can I learn how to bill insurance?
A. You can visit the EI CBO website at
www.eicbo.info. They have information on the billing process. CBO also has assistance by phone for billing.

Q. I am going to be working under another DT for short period of time and will be sending in additional paperwork to be put in the system as an independent contractor also. Will I need to get an LLC?
A. You should consult with an accountant or an attorney regarding the best way for you to establish your independent enrollment.

Q. How do I remove my name, position from a company that I no longer work for. 312/296/7478.
A. Send or fax a letter to Provider Connections stating the name of the organization for which you no longer work and your last date of employment. Provider Connections will forward the necessary documentation to the EI CBO.
