Appendix F: Agreement
Use of Videotaped Lectures
This agreement is to be used when a regular class is videotaped
for use at a future date if the class offered only on campus or if it
is simultaneously transmitted to an off-campus site. This agreement does
not cover tapes produced by Educational Broadcasting and Independent Study,
School of Extended and Continuing Education, for use by that office or
for lease to another institution.

Instructor/faculty member:
- Western Illinois University is the owner of the
videotapes for the above class.
- Western Illinois University may not use the tapes,
or provide the tapes to any other party, as part of an independent study
or extension course without the faculty member's permission.
- Western Illinois University may not use the tapes,
or provide the tapes to any other party, as part of an on-campus class
without the faculty member's permission.
- The faculty member may not use the tapes to teach
a regularly assigned class for which compensation is being paid and
regular credit units are being assigned without approval from Western
Illinois University.
- The faculty member may not use the tapes as part
of his/her own independent study or extension class for which no regular
class meetings are scheduled without prior approval of Western Illinois
University.
- The faculty member may not use the tapes to provide
instruction at any other institution without the permission of Western
Illinois University.
- Waivers of this agreement, or portions of this agreement,
must be obtained in writing and signed by both the faculty member and
the Provost, or the Provost's Designee.
