Diagnosticians
Documenting a disability for the purpose of receiving academic accommodations in a post-secondary setting can be tricky if you are not familiar with higher education disability law. Furthermore, each institution establishes its own documentation requirements, and a document that establishes eligibility for accommodations at one institution may not at another. This can be frustrating to students who are denied accommodations for insufficient documentation and to the diagnosticians who spent valuable time drafting letters or reports.
To assist diagnosticians in providing Western Illinois University the information necessary to support accommodation requests, Disability Resource Center created the following fill-in-the blank forms. Just click on the appropriate disability category, fill in the information, and fax it to 309-298-2361 or mail it to Disability Resource Center, 1 University Circle, Macomb, IL 61455.
