Verification Forms

You will be contacted by WIU email if your FAFSA was selected for verification. You may also check STARS for your Financial Aid Status.

If selected, a Verification Worksheet and federal tax transcripts are required to determine financial aid eligibility. Please follow the instructions included with the worksheet.

Please do not send worksheets, tax returns, or tax transcripts unless the information is requested from you. Copies of federal tax returns NO LONGER ACCEPTED - please reference worksheet instructions .

Forms require Adobe Acrobat Reader. If you require any of these forms in an alternative format, please contact the Financial Aid Office at Financial-Aid@wiu.edu.

Fall 2013 - Spring 2014 Academic Year

If you and your parents are able to retrieve your IRS tax data directly from the IRS when completing your FAFSA, your chances of being selected for verification are greatly reduced! For additional details, watch the 2013-14 IRS Data Retrieval video tutorial. Please reference the Availability of 2012 Tax Information chart to determine when your 2012 tax information should be available for retrieval or request! 

To print your personalized 2013-14 verification form, please:

  1. Log on to STARS
  2. Select "Financial Aid Information" from the menu
  3. Click the "2013-14 Academic Year" button
  4. Click the "Verification Form" link listed in blue at the top of the screen
  5. Print your personalized verification form

 

If your FAFSA was selected for verification and all required documents were submitted to the Financial Aid Office: Expect your award letter or a request for additional information/clarification no sooner than:
February 26-March 11 April 12
March 12-25 April 19
March 26-April 8 April 26

 

Fall 2012 - Spring 2013 Academic Year
