PLEASE NOTE: IF YOU RECOGNIZE ANY ABOUT THE NEXT PAGE, YOU HAVE BEEN
IN THIS STUDY BEFORE. PLEASE DO NOT PARTICIPATE AGAIN. THANK YOU.
Welcome to the memory study. I am Bem Allen, Ph.D. Professor Emeritus
of
psychology
at Western Illinois University (WIU), USA. First, I want to thank you
for
your willingness to help us learn more about memory. Second, I want to
indicate that you will have an opportunity to learn more about your
memory
and about memory itself. Third, I would like to ask you for your
consent
to be in this study and allow us to use your responses, without, of
course,
you being identified with them in any way. Below, I will ask you to
"sign" a consent
form (with your nickname so you can get feedback). IF YOU
ALREADY
HAVE A NICKNAME, MAKE UP A NEW NICKNAME FOR USE IN THIS STUDY,
ONE
THAT YOU WON'T FORGET. IF YOU DON'T HAVE A NICKNAME, MAKE ONE UP THAT
YOU
WILL NOT FORGET. Please keep your nickname in mind, as you will need it
during the course of the experiment.
In fact, it is absolutely essential
that
you use your nickname.
For every memory test, the first item
you record, before your recollections, will be your nickname. You
should
know that this research has been approved by the WIU Human Subjects
Committee,
that we know of no harmful effects associated with participation, that
information submitted via http is not 100% secure, and that the study
will
take about 25 minutes. If you have any questions concerning your rights
as a subject contact IRB-Administrator@wiu.edu.
So please decide whether to participate now... if you decide "yes,"
use the window below to indicate your consent. You must be 18 years old to participate. You
may withdraw at any
time without penalty and will be given an opportunity to reaffirm your
consent at the end. The experimenter can be contacted at mfbpa@wiu.edu.
After you have submitted your consent you will
return
to this page to begin the experiment. Scroll down now.
