Authorization for a student to checkout library materials for music department faculty

By submitting this form, I hereby authorize the student entered below to checkout library materials on my behalf. Furthermore, I agree to abide by the following conditions:

• All materials are to be checked-out in my name and I take full responsibility.
• Periodicals may be checked out for 2 days.
• Other library materials may be borrowed for the normal checkout period.
• Only circulating items are available for checkout (no Reference, etc.).
• The student will locate and bring the items to the Circulation Desk for checkout.
